Fellow Nomination Form

Thank you for nominating a Wisconsin Academy Fellow.

Our nomination process for our next selection process is now open. For consideration, nominations will be accepted through the Wisconsin Academy website until autumn 2017.
Please note: over the course of the next year, we will be making upgrades to our online process.

BEFORE YOU BEGIN

Make sure you have all the required information and materials as outlined in the instructions below.

Form Instructions

The Fellows nomination is divided into two (2) sections. Each section must be completed before moving on to the next. Required fields are marked with an asterisk (*) in the form. The form must be completed in a single session.  It is NOT possible to save progress and return. There are also size and naming requirements for the support materials, see details directly above "Required Materials."

Section 1: Contact Information
You will provide the required contact information onlined below for your Nominee and for you and any additional Nominators (up to 5 total).

Section 2: Nominee Materials
Reqired and supplemental materials are documents and/or weblinks that will be uploading onto the Wisconsin Academy website.

NOMINEE MATERIALS

Please note the size requirements for each file. If a document is too large, you will not be able to submit the nomination. Nominee material must also be titled as follows, in order for the Selection Committee to be able to review the materials. Your nominee's full name should replace the generic "NomineeName" in the brackets next to each requested file.  Click here if you need help with how to rename your files.

3. Required Materials
Accepted Format: pdf
  • Nomination Statement (4 parts), 2 MB max -- [NomineeName_FY18_NomStatement]
    1) Citation, 50 words or less: A succinct statement such as could be used at the time of induction of a successful candidate.
    2) Awards and Honors: Please list significant awards and honors received by your nominee.
    3) 250-word Statement: Summarize the main reason for the nomination. Achievement in some area is the main criterion, but interests beyond one's specialty and service to the state and society at large may also be consideration.
    4) Major Achievements: Describe in more detail the work for which the candidate deserves to be elected as a Fellow.​​

How to Rename a File

Microsoft Windows
Method 1: Highlight the file name by clicking it once. Right click with the mouse and select "Rename" from the menu. Type a new file name.
Method 2: Highlight the file name by clicking it once. Press F2 on the keyboard. Type a new file name.
Method 3: Highlight the file name by clicking it once. Click "File" at the top of the window and select "Rename" from the menu. Type a new file name.
Method 4: Open the file with the program used to create it. Click "File" at the top of the window and select "Save As" from the menu. Type a new file name in the save window.
 
Mac OS
Method 1: Highlight the file name by clicking it once. Press "Return" on the keyboard. Type a new file name.
Method 2: Highlight the file name by clicking it once. Hover over the highlighted name with the mouse until the name pops up. Click once more with the mouse. Type a new file name.
Method 3: Open the file with the program used to create it. Click "Duplicate" at the top of the Window.  Save the duplicated file with a new name.
